Az uralkodó aláírásával, valamint Szegedi Ferenc Lénárd (1614-1675) váci püspök és Orbán István ellenjegyzésével, a plicán kihirdetési záradékkal (1670, Hont vm.). Kék-arany sodraton uralkodói viaszpecsét töredékével. Hátulján későbbi feljegyzések. /
